2. Fill in your information. Have your name, mailing address, birthdate, email and phone number ready. Include your apartment or unit number if needed.
3. Review the marketing checkbox. The form offers enrollment in Optic Obsessed Perks and promotional communications. Read the wording and choose your preference.
4. Double-check and submit. Follow any confirmation instructions on Colgate’s page. A form submission does not guarantee a bundle if supplies have already run out.
