- Have your golf details handy. The form asks for contact and address information, date of birth, gender, handicap, your usual golf ball and playing frequency, plus the equipment in your bag.
- Answer accurately. Check your club models before starting. Don’t invent a handicap or choose equipment you don’t use to try to qualify.

The signup page does not specify a guaranteed quantity, model, delivery date or closing date. Read any individual invitation carefully before agreeing to participate.
Is the Titleist testing program real?
Yes. This form is on Titleist’s own website. In an official staff response about test balls, Titleist confirms that it sends golf balls for testing and directs members to this same signup page. That older response explains the program’s background; it is not a promise of a shipment today.
Titleist’s current Team Titleist community page also lists product testing among its member experiences, alongside clinics, focus groups and outings. Think of it as joining a golf community with possible research opportunities—not claiming a retail giveaway.
